Collaboration is at the heart of our approach, both within the council and with our partners, as we strive to enhance and safeguard the well-being of the people in Staffordshire. Actively involving and empowering our communities is crucial to supporting our efforts in providing assistance to all residents who are elderly or vulnerable adults.

Individual inquiries and concerns receive swift attention, ensuring that older and vulnerable adults receive the care they require and are kept safe from harm. We actively manage the care and support market to ensure its availability where needed and that it is affordable for our residents.

We are committed to working collaboratively with partners from across the independent, private sector across Staffordshire to build a more resilient care sector that enables more effective recruitment and retention in adults’ social care based on person-centred and innovative ways of working.

Richard-Harling

The Director for Health and Care, Dr Richard Harling, works with Elected Members and public, private and third sector partners and communities to harness the county’s resources to deliver improved health, prosperity and happiness for local people and communities. He has full leadership accountability for the transformation and delivery of business plans for Adults Social Services and Public Health across Staffordshire.

Working in partnership with Midlands Partnership Foundation Trust

Here at Staffordshire we work in partnership with Midlands Partnership NHS Foundation Trust to support our residents across Staffordshire.

MPFT provide physical and mental health care, learning disability, and adult social care services across Staffordshire, Stoke-on-Trent, Shropshire, and Telford & Wrekin. In addition, they provide services on a wider regional and national basis including perinatal, eating disorders, and forensic services. They also deliver out-of-area sexual health services, and our Inclusion service offers psychological and drug & alcohol services, in the community, and in prisons.

Our service operates in partnership to empower individuals needing support to maintain and enhance their independence. We take a strength-based and person-centred approach, focusing on outcomes that align with statutory requirements. When delivering services, we prioritise cost-effective solutions that not only meet a person’s needs but also improve their overall well-being.

Adult Learning Disability

We are dedicated to maintaining the highest social work standards, empowering individuals with learning disabilities and Autism to live life on their terms.

Our teams collaborate closely with individuals and their caregivers, conducting strength-based assessments and creating outcome-driven support plans. These plans align with their assessed social care needs, promoting independence and fulfilling aspirations, as per the Care Act 2014.

The countywide Preparing for Adulthood Team assists young individuals (14-25) with learning disabilities and Autism. We work with their families, caregivers, and colleagues in education and children’s social care, conducting strength-based assessments and developing outcome-focused support plans. Our aim is to ease their transition into adulthood, empowering them to achieve their dreams and ambitions.
Prisons & Approved Premises Team

We are a dynamic team consisting of social workers and occupational therapists providing social care support in the 7 prisons and 2 approved premises in Staffordshire. Staffordshire has the largest prison population in England and Wales with about 5,400 prisoners.

Adults in prisons and approved premises are entitled to receive the same access and provision of adult social care services as those within the local community. We work in partnership with the multi-disciplinary team in the prison to improve outcomes and enhance wellbeing of the adults with need for care and support in the custodial settings.

The Prisons and Approved Premises team utilises a strength based approach to provide care and support services to adults in custodial settings across the full spectrum of need. Ensuring that adults in custodial settings have the care and support options, including equipment and assistive technology, they need to maximise their independence whilst in prison. We also seek to support adults as they prepare to leave prison and return to a community setting.
Safeguarding Service

The adult safeguarding service is responsible for meeting the duties of the local authority under Section 42 of the Care Act (2014). We have two teams that make up the service – The Staffordshire Adult Safeguarding Team (SAST) and Adult Safeguarding Enquiry Team (ASET).

SAST

This team is responsible for receiving all safeguarding concerns that are received across the county and for all disciplines of adult social care. Here we make decisions as to if the concern requires further enquiries under section 42 of the care act. To do this our team, which consists of Advanced Practitioners (qualified social workers) and safeguarding assistants gather information from the adult, carers and partner organisations to consider risk and appropriate next steps.

We work within the Multi-Agency Safeguarding Hub (MASH) working alongside police and health colleagues to effectively share information at the earliest opportunity to safeguard adults who may be at risk of or experiencing abuse.

The team also consider and monitor concerns which involve Person in Positions of Trust (PIPOT)

ASET

This team is responsible for making enquiries under section 42 of the Care Act, in line with Making Safeguarding Personal, for all high risk and complex safeguarding concerns. They are a dynamic team with social workers, nurses and other registered professionals working alongside (co-located) with police colleagues.

The team are also responsible for managing providers under our Large Scale Enquiry (LSE) process and monitoring other services that are at high risk. Partnership working is essential both with our internal quality Assurance and PIRT teams but also with health provider, ICB and other agencies.
Adult Social Care Core, Support & Improvement

We commission adult social care arrangements such as assessment and case management for older people and people with physical and sensory disabilities, for adults with mental health conditions, financial assessments and direct payment support and payment card providers.

The team also lead on performance management arrangements for both commissioned and in-house adult social care assessment and case management: ensuring that there is a consistent approach to performance management for all client groups, e.g. assessments and reviews.

Our business support team provides support for Adults Learning Disabilities Teams and our care systems team acts as the liaison between service areas and system providers, providing advice on developments, emerging opportunities and functionality within our systems to improve and support business processes.
Care Commissioning
Care commissioning are responsible for the following functions within Health and Care:

Commissioning, shaping and developing Adult Social Care services and managing brokerage of those services.

Ensuring quality and effectiveness, long term sustainability, good value, effective outcomes and measurable improvements in quality and productivity in the services the Council commissions for people with care and support needs.

Co-producing, developing and implementing multi-agency strategies and plans, which achieve priority outcomes for Staffordshire residents.

Implementing a strategic, commercial and outcome-focused approach to management of current and future adult social care providers and leading market/provider development activities.

Older People and Physical Disabilities Commissioning Team

We provide social care support to older people and working age adults with a physical or sensory disability. We give on-going social care support to approximately 10,000 older people and 1,200 younger adults with a physical disability each year.

All Age Disability and Mental Health Team

This team focuses primarily on improving outcomes for people with Learning Disabilities, Autism, Mental Health aged 18-64 and Carers of all ages in Staffordshire. The numbers of individuals we commission services for are lower than for the Older People and Physical Disability Team, but the care needs in this cohort mean that they may require support over longer periods and they may also require more intensive support.

Quality Assurance Team

The Quality Assurance Team is responsible for the monitoring of Care Quality Commission (CQC) regulated adult social care services. This includes care homes, home care providers and supported living providers.

The team collates and analyses information to identify which services require enhanced monitoring and support. This includes visiting services and creating action plans, along with sharing resources and guidance to support the provider and drive improvements.

Provider Improvement and Response Team (PIRT)

The Provider Improvement and Response Team is jointly funded between Staffordshire County Council and the Staffordshire and Stoke-on-Trent Clinical Commissioning Groups and is comprised of nurses and officers.

We work with residential and nursing care home providers who require more intensive support, based on escalated level of concerns. These care homes can also benefit from the clinical expertise in the team.

Like with the Quality Assurance Team, this includes visiting services, creating action plans and sharing resources and guidance.
Care Market Development Team

The Care Market Development Team works with Staffordshire’s care organisations to recruit, retain and develop highly skilled staff. We support those seeking employment in the sector and employers who are seeking staff. We also plan and facilitate access to a wide range of sector-specific training and events to staff at all levels, in addition to signposting the social care sector to a wide range of information, guidance and resources.

Brokerage

The Brokerage Service plays a crucial role in sourcing care and support services to meet adult social care assessed needs while achieving best value for the taxpayer. We link in with Social Care Assessors and Social Workers, individuals with assessed care needs, their family members, carers or representatives regarding service offers to support a timely start to their care and support services.

We use digital systems where possible to source care and support, but for some services we will also contact providers directly to negotiate both price and quality. This includes residential and nursing care, home care, extra care, day care, supported living and carer support. The team works closely with the Council’s commissioners, commercial unit and Quality Assurance Team, so that any gaps in market capacity can be quickly identified and resolved. We also link closely with care providers, giving them as much information on future and present demand as possible.

We provide care and support for adults with complex needs, including learning disabilities. We offer services around the county including residential care homes, supported living, respite, day centres and home-based support.

Public Health
In Public Health, we have a clear leadership role in driving the health and wellbeing agenda and will provide expertise and specialist services where needed. However, the capacity to create and maintain good health for the population lies across the whole system, and our organisation plays a significant part in this.

While Staffordshire is a relatively healthy place to live, there are many challenges, and pockets of very poor health. To achieve better outcomes for our residents, we need to improve healthy life expectancy, reduce health inequalities, and reduce demand on adult social care and healthcare services. Our team focuses on the prevention of poor health: we work closely with communities and partners to ensure that we are providing guidance and support across Staffordshire. We use data and intelligence to target areas of deprivation or greatest need, to ensure that resources are directed to those who need it most.

Health in all we do (e.g., embedding health and wellbeing in Local Plans, health impact assessment of new strategies)

Health Protection – Working to prevent or reduce harm from infectious disease and environmental hazards and includes immunisation (vaccination) and screening programmes

Healthy Behaviours – Working with a range of stakeholders to develop strategies and commission services/programmes to prevent and reduce poor health outcomes in a number of priority areas

Supportive Communities – Supporting vulnerable adults to stay healthy and independent for longer by working collaboratively with local communities, the workforce, and wider partners, to deliver a preventative approach to adult social care
